Patient Forms & Information

We appreciate your choice of Vista Eye Specialists for your complete eye care. During your first visit you may expect to spend approximately one to two hours in our office. Dilation of your eyes may be required for the examination. Once your eyes have been dilated, the dilation effect may last one to three hours. If possible, try to arrange for a ride home if you feel you may not be comfortable driving after having your eyes dilated.

To save some time and make your visit more relaxed, here are the patient forms we will need for your appointment. Please open, print, and complete the appropriate forms for your visit / appointment. By completing these forms prior to your appointment, you will be better prepared for any questions that arise. Please be sure to bring these forms with you to your appointment.

Also, please bring a list of any medications and eye drops that you are currently using, along with your current eyeglasses or contact lenses. Be sure you have all of your insurance cards and identification cards with you when you come to the office.

Applicable co-payments and deductibles will be collected from you for all insurance plans with which we participate. Payment is requested at the time of the office visit for any co-payments or non-insurance services and may be made in cash, by check or credit card.
